If You Are Taking a GLP-1, Nutrition Is Not Optional

These medications work. What determines whether the result lasts, and what you lose along the way, is everything you do alongside them.
If you are taking a GLP-1 medication, or you are about to start, or you have taken one and regained the weight afterwards, please read to the end.
These medications work. That is not in dispute. The question is what happens around them.
The point almost everyone misses
Every clinical trial of GLP-1 receptor agonists included a lifestyle intervention. Not medication alone. The research design itself assumed nutrition and lifestyle support alongside the drug.
A large systematic review and meta analysis of forty seven randomised controlled trials examining these medications for weight, BMI and waist circumference reflects this consistently. The women who get the best long term results are the ones combining medication with a structured nutrition and lifestyle plan.
That is not an opinion about willpower. It is what the evidence base actually looks like.
What you are protecting against
Appetite suppression is the mechanism, and it creates a specific risk: eating far less of everything, including protein.
When intake drops sharply without adequate protein and without resistance training, a meaningful proportion of what you lose is muscle rather than fat.
That matters enormously, because muscle is:
- Where glucose goes, and therefore central to insulin sensitivity
- The main determinant of your metabolic rate
- What protects your bone density
- What determines how your body handles food after you stop the medication
Losing weight while losing muscle sets up the regain. You come off the medication with less metabolic capacity than you started with.
What to do alongside it
Eat a high protein, moderate carbohydrate diet. Protein at every meal, without exception, even when you are not hungry. Particularly when you are not hungry.
Do resistance training. This is not optional either. It is the signal that tells your body to keep the muscle it has while you are in a deficit.
Know which supplements you need. Reduced intake means reduced micronutrient intake. Iron, B12, vitamin D and others need attention, ideally based on testing rather than guessing.
Have someone following you. Someone monitoring what is actually happening, adjusting as you go, and watching your body composition rather than only your weight.
The regain question
The women who regain are almost always the women who did not know what to eat or how to behave during the process. When the medication stops, nothing has changed about their habits, their muscle mass or their understanding of their own body.
The women who hold their results are the ones who used the period of reduced appetite to build a structure that works without the drug.
That is the whole difference.
What I do and do not do
I am a clinical dietitian. I do not prescribe these medications and I do not manage them. That belongs with your doctor.
What I do is the nutrition and lifestyle work that determines whether the medication produces a durable result: blood sugar regulation, adequate protein, muscle preservation, anti inflammatory nutrition, gut support and supplement guidance appropriate to where you are.
Medication and nutrition are not competing options. In the trials, they were never separate.
